Simulation of first level health care facilities to reduce patient flow time

Abstract

This paper presents research on simulations of first level health care facilities, namely at Community Health Centers that are managed by the government and at the Primary Clinic which is managed by the private sector. The objectives of this study include identifying the patient flow time, the location of bottlenecks, and determining scenarios to improve system performance. The simulation model was developed using a discrete event simulation approach, after the objectives were determined, the study continued with data collection in the form of flow and volume of patient arrivals and service times in each location. By analyzing statistical data from 30 replications, it shows that there are differences in characteristics between Community Health Centers and Primary Clinics, namely at the location of bottle-neck, and the performance of Primary Clinics was better than Community Health Centers, with patient flow time, respectively 2516.23 seconds and 4881.03 seconds. However, after improvements were made in the bottle-neck locations at the Community Health Center and Primary Clinic, the patient flow time was almost the same, 1787.81 seconds and 1731.25 seconds, respectively. Conclusion, when the bottleneck location can be identified, improvements can be made to reduce the patient flow time of the two health care facilities, and even show relatively the same time.
