As for the purpose of this study is to analyze and study the problems of medical record information systems on Health Center and produce a medical record information system, these problems can be over come by using a medical record information system that was develop with the help of RFID technology, PHP and MySQL programming languages that process patient medical record data and storage media into the database. With the existence of such information systems can create more effective performance, efficient and minimize errors due to human error.
